Families and children with chronic kidney disease have to cope with complex medication regimes which change constantly. The Royal Hospital for Sick Children in Glasgow has developed the ‘Renal Medicines Booklet’ to provide easily understood written information about their medicines to patients and families of children with chronic kidney disease. Initially aimed at kidney transplant patients, it was soon recognised that this resource would benefit all patients attending the renal unit, and now everyone receives the booklet, either just before discharge if they have been an inpatient or when they are seen as an outpatient at a renal clinic.

Shires Health Care
The Joined Care Programme by Shires Health Care aims to improve understanding of the use of medicines and address the common problems relating to medicines use in the elderly and other vulnerable people. A series of 2 hour medicines management workshops, run by a community pharmacist, were delivered to a range of staff, carers and people who are directly involved in patient medication identified by care homes and service managers. The workshops also provided a forum whereby participants were able to share examples of best practice and were able to become more familiar with the local pharmacy service for future working relationships.
